Office Visits

If this is your first visit to LNSA, please arrive 20 minutes early to complete a patient information form and a medical history.

BRING: your social security number, all insurance cards, and, if it applies, your HMO referral or insurance claim form.

Your appointment time is reserved for you, and we do our best to stay on schedule. When emergencies arise, we greatly appreciate your patience, and regret any inconvenience. If a delay is going to be for a significant amount of time, we'll tell you, and you may choose to reschedule. If you must cancel or reschedule, please call us as soon as possible.

Second Opinion

The staff of LNSA recommends a second opinion from a qualified physician, especially when surgery is indicated. With spinal surgery, many insurance companies require a second opinion. Should you need or want a second opinion, we will gladly give you a comprehensive list of qualified physicians.
