BRING THE REGISTRATION AND MEDICAL HISTORY FORMS. These forms will have been mailed to you. If time permits, prior to your appointment. You can fill them out and bring them along.

You can fill out your forms through this web site, print them out, and bring them along. |

Having your forms filled out when you arrive will save time and eliminate any anxiety that can result from completing forms in the waiting room. |

If you cannot fill out the forms prior to your appointment, plan to arrive early, to allow time to do so. |

BRING YOUR SOCIAL SECURITY NUMBER AND ANY APPROPRIATE INSURANCE CARDS. |

BRING ANY APPROPRIATE X-RAYS, MRI OR CT SCANS, which are very helpful in making your initial consultation productive. |

BRING A LIST OF THE MEDICATIONS you are taking, with the specific names and dosages. |

BRING THE OCCUPATIONAL INJURY REPORT, if Worker's Compensation is the primary insurance. |
